The Accessible Information Standard aims to ensure that patients and/or their carers who have a disability or sensory loss receive information in a format they can read and understand. For example, a patient may require large print, braille, email rather than telephone communications, or professional communication support (i.e. British sign language interpreter).

Individuals most likely to be affected by the Standard include people who are blind or deaf, who have some hearing and / or visual loss, or people with a learning disability.

At Poole Town Surgery we want to make sure that we are communicating with you in a way that is easy for you and that you can understand.

Help us to help you by letting us know if you:

• Need information in a specific format? e.g. large print or easy read
• Do you need to receive information in a particular way? i.e. text or email
• Do you need someone to support you at appointments? e.g. sign language, interpreter or a carer or friend?
• Do you lip read or use a hearing aid?

We will do our best to ensure that your needs are met and with your permission record this in your file and share the information with other NHS and adult social care providers.
